With phpbb 2.0.11 including the visual confirmation code in profile_add_body.tpl on many themes require editing.
I was reading on phpbb.com and it was stated that:

All styles in phpbb.com styles database do support it. All styles that don't support it were removed from database few month ago.

Just thought I would let you guys know.
Note: I tried to post the code changes here, but it was not allowed. You can see the change by downloading phpbb 2.0.11 and looking at profile_add_body.tpl. The section for username is changed and a section for the visual confirmation code is added.
