Hi,
I created a module, using toolkit 3. I put an image and some text as a test. I placed the image in the module folder along with index.php.
The text showed up, but the image has a red x in it's place.
(The image was a jpg.) I am using PHP-Nuke 7.4

Check the path to the image and make sure it is valid.
Right click the broken image and select properties.

That was the problem. I apoligize, that was a silly mistake.
