The only time I have seen the _ERRO upon registration is when the wrong Your Accoutn module for your system is used.

Make sure youy are using the correct Your Account module . If you run phpBB ported forums, then you need to use the Your Account module that came with the phpBB port. Along with the modules.php file that came with it also.
