I will try installing it again. The only change I recall making was to the header file. However, I could have changed something else by mistake.

That message means that your Forums are using a template that is not in your modules/Forums/templates directory or doens't have a theme_info.cfg file This looks like you installed the theme incorrectly. All you do is upload the theme to your themes folder as-is. DO NOT CHANGE anything. Then just select the theme from your account page. Do not change ANYTHING on the Forum side.
