Are you asking for a link to that module for download?

If so, I'm not sure where that particular module is available. You may want to ask a staff member from the site you got that block from where they got it.

What duke means is that's a custom block (doesn't look like there's a login there though). There are a ton of them out there, but I'm not sure which one that is.

The login block you currently have is the default one that comes with pretty much all nuke packages. You're looking to replace it with a custom block so you'll have to search to find one you can live with or as I stated earlier, go to the site you saw that block on and ask them where the dl'd it from.
