How to stop "Too Many Connections"

Hello

I always get this problem with MySQL DB

Too many Connections

I have read on how to stop this but I just dont understand where I am meant to go and what I am meant to do. If you could give me some help I would appreciate. It occurs normaly once a week. I am PHPNuke 6.0 using toms port if that helps.

Can anyone help me with this because it really is ruining the site and it will often mean the site is down for hours apon end
